2. Enter the store number from the top of your receipt into the first blank box.
3. Use the date picker widget to select the date that you visited Smashburger.
4. Pick the time of your visit from the drop down menu.
5. Enter the check number in the last blank box. This order number will be located beneath the receipt date.
6. Click on the “Start” button.
7. Answer the list of questions about how satisfied you were with your trip to Smashburger.
8. Enter data about your age, gender, and income range to help Smashburger learn more about marketing to your demographic.
9. Provide your notification information if the survey invites you to register for a sweepstakes program.
10. Submit the survey.
11. Copy the validation code onto your receipt.

      If you would like to speak with someone at Smashburger directly you can either call their customer service department, or use the store locator to find the contacts of the specific location you visited, and speak with the managers of that location.

    I was visiting my grandmother for Christmas and my mother and I got smash burger for lunch. I went to pick it up and I’m glad that I did. It was in Port Washington, L.I. Store # 1561. First of as soon as I walked in I was pleased the restaurant was clean bright and welcoming, I then got to the counter and the woman Angela was clearly very good at her job and very friendly as well. I then saw after she helped everyone on line, quite quickly I might add, she then immediately entered the dining area and began helping to clean up. I just hope that you’re aware of how good some of your employees are because I’m a business owner and am fully aware of what a bad employee is and what they can do to your business. I definitely enjoyed my experience and will absolutely return to your establishment next time I’m in town. Thank you it made my visit to my grandmothers just that much better.
